Transition Planning 

The Community Clinic offers Transition Planning for youth and adults with disabilities (ages 16-22) to prepare clients for adulthood. During two individual sessions, assessment and intervention in the areas of self-determination, career readiness and interest, independent living skills and recreational interest are provided. The clinic concludes with a final client-led transition planning meeting with his/her clinician, family members, regional caseworkers and other stakeholders.

  • Cost: No charge for Transition Planning Clinic
  • Ages: This service is provided for clients ages 16 to 22 years old.
  • When: Assessments are conducted once a year (April thru May). For specific dates and times, please visit our "Services" page and scroll down to see our Clinic flyer.

    Although we accept applications year-round, the best time to submit an application is two months before the start date of the service you are interested in. (However, please note that admittance is based on first-come, first-served basis and the level of need).
