Fee Reduction Guidelines

The Community Clinic for Counseling & Educational Services strives to make services easily accessible to individuals/families in financial need by providing scholarships/ fee reductions. Fee reduction availability and amounts vary from service to service. 

Submit the completed Fee Reduction Request Form along with the necessary documentation to the clinic office for consideration.

Student qualifies for for free or reduced lunch Proof of qualification on district/school letterhead
Family Income at or below 2023 HHS Poverty Guidelines (see chart below)Copy of pay stub, 1040 form or other tax document.

The Federal Register notice for the 2019 Poverty Guidelines was published January 19, 2023.
