Fee Category IV

Description

Fees, other than Category II or III fees, paid to receive materials, services, or for the use of facilities provided by the university; and fees or deposits to reimburse the university for additional costs resulting from dishonored payments, late submissions or misuse of property or as a security or guarantee.

Examples

Late registration fee, late payment fee, missed deadline fees, chemistry breakage, lost keys, etc. and Enrollment Deposit Fee.

Authority

The president is delegated authority for the establishment, oversight and adjustment of Category IV fees.

Responsibility

  • AVP for Financial Management - The AVP for financial management will consider proposals and make a recommendation to the president.
  • President - The president will make a determination on Category IV fees after consideration of the revenue and expenditure plans associated with the fees, and will notify the fee advisory committee of his or her decision.
  • Fee Advisory Committee - Receives notification after approval by the president.

Instructions

  1. Determine the sub category (as defined below) for the proposed fee action.

  2. Fill out, review and submit the on-line form.

  3. Click on the request access link provided after submission and print the submitted request.

  4. Optional - Attach any supplemental materials that provide further justification (i.e. detailed costs, quotes, comparison rates, etc.).

  5. Obtain signature approval for the information submitted on the form by each of the people included in the Authorization to Submit Request section of the form.

  6. Route the original form with approval signatures and supplemental materials (if applicable) to the University Bursar, BH-155.

  7. The Bursar will prepare the proposal for review by the AVP, Financial Management.

  8. The AVP, Financial Management in consultation with the Chair of the Student Fee Advisory Committee will consider the proposal and make a recommendation to the President.

  9. The President (or designee) will report the results to the SFAC.

  10. The President (or designee) will report the results to the requestor.
