Please review the following information carefully. Please note that there are several forms and supporting documentation you will need to submit to finalize the hiring process. Inability to submit all required documents within three business days from your first day of employment will impact your employment status. In most cases, the first day of employment is considered the day you attend the New Employee Orientation.

Step One: Please review following documents.

You will have the opportunity to complete all required documentation at the New Employee Orientation. All forms and supporting documentation must be submitted within three business days from your first day of employment. You may choose to submit the required documentation prior to your orientation date at the Human Resources Office. 
 
The Human Resources Office is open Monday through Friday from 9:00AM to 4:00PM.
NOTE: Incomplete documentation will be returned and will not be processed.

2. Federal and State Income Tax Witholding Forms
 
In addtion to completing the above forms, please complete the following:
a. Printout of your current CSULB Class Schedule from your MyCSULB portal. Please select the "List View" option because it includes your first and last name.
b. Your CSULB Student Identification Card
c. Bring in your Social Security card for an HR Representative to verifiy for payroll tax reporting purposes.
d. Original documents must be submitted to the HR Office for I-9 employment authorization. Copies cannot be accepted. (Please see page 3 of the Form I-9 for a list of Acceptable Documents.)
e. It is not necessary to print the Form I-9 Instructions (pdf). Please use them as a reference for help while filling out the Form I-9.

Step Two: Please Confirm Your Attendance for New Employee Orientation.

Human Resources will contact you to collect your basic onboarding information, and will confirm your orientation date over the phone. A formal confirmation email will also be emailed to you prior to your orientation date.
