Student Athletes

All new, incoming athletes must show proof of the following vaccines:

Required Immunization Required Documentation
Measles, Mumps and Rubella (MMR)

All incoming students born after 1956

  • 2 doses on or after 1st birthday
Meningococcal Conjugate (Serogroups A,C,Y & W-135) (MenACWY)

All incoming students through 21 years of age

  • 1 dose on or after the 16th birthday
Hepatitis B

First-time enrollees at California State University campuses 18 years of age and younger.

  • Full Immunization series
Varicella (Chicken Pox)

All incoming students born after 1979

  • 2 doses on or after 1st birthday
Tetanus, Diphtheria, and Pertussis (Tdap)
  • 1 dose on or after the 7th birthday.
Tuberculosis (TB) risk questionnaire All incoming students must complete a Tuberculosis (TB) risk questionnaire. Incoming students who are at higher risk for TB infection should undergo either skin or blood testing for TB infection within 1 year of entry into CSULB

Please note that titers will only be accepted for MMR, Varicella, and Hepatitis B. If you have both an immunization and titer, please upload the immunization record only.
