Release Forms

All release forms below can be found in the documents folder under Release Forms

• Release of Liability 
  • All persons participating on any production must complete a Release of Liability. This includes cast, participants, crew, and any other person involved in the activity. Release of Liability Waivers must be completed prior to setting on set.
  • If needed, a Spanish version of the Release of Liability is available.
    • Students and Employees of the university must fill out the English version. Only non-CSULB participants who speak Spanish should fill out the interpreted version. 
• Personal Appearance Release
  • Any Participant, On-Screen talent or extra seen on camera must complete the Personal Appearance Release.
