Printmaking Student Safety Information, Quizzes and Forms
All students enrolled in Printmaking Classes in the School of Art must complete the steps outlined below in order to use the facilities and equipment provided in the Printmaking Spaces.

- Please fill out and sign the Printmaking Safety Instruction Verification and Agreement Form. (Required for ALL students)
- Please review the Printmaking Health & Safety Information.
If you plan on utilizing Extended Hours in the printmaking facilities outside of class time and scheduled supervised times, the following steps must be completed before application review:
- Please fill out and sign the Printmaking Safety Instruction Verification and Agreement Form. (If you have already completed the Printmaking Safety Instruction Verification and Agreement form, go straight to your quiz below!)
- After reviewing the Printmaking Health Safety Information, please check and confirm your understanding of the information with the Extended Hours Safety Quiz.
- You MUST receive a score of 100% on the Safety Quiz
- IF your score is lower than 100%, please review your submitted quiz answers, look over the information once again to learn the answers to the question(s) you missed and Take the Quiz again. Resubmit your answers and continue repeating this step until you have received a score of 100%.
- There are no penalties for retaking the quiz.
- After receiving 100% test score on your Printmaking Safety Quiz and submitting the Printmaking Safety Instruction Verification and Agreement Form, please fill out the Extended Hours Authorization Form.
After review of your required forms and receiving 100% on the printmaking quiz, an approval email will be generated for your Extended Hours Authorization Access.
N95 Dust Mask
If you believe you may use a disposable N95 Dust mask or know you will wear one, go through the Volunteer N95 Dust Mask Waiver before use. This waiver is required every semester and applies to all your enrolled courses in which an N95 is used.
