CSULB COVID-19 Pre-Arrival Health Screening for Guests Attending Events on Campus:
Students, faculty, staff, and external guests must complete a pre-arrival health screening questionnaire each day before coming to campus. The online questionnaire for students, staff, and faculty is available on the single sign-on site.
External guests visiting campus who do not have access to the COVID-19 prescreen in the Campus Single Sign-On (SSO) service should use the Visitor COVID-19 prescreen.
Entry into campus spaces will be contingent on you displaying the current day's "green checkmark" on your mobile device resulting from clearing online health screening.
If the result of your pre-screen questionnaire is a "red X," you will be subject to additional questions. If necessary, you will be contacted for follow-up. Do not go to campus until you have been cleared.

This event is part of the CSULB Arts Festival 2022: Together, a series of arts and culture events collectively presented by the College of the Arts (COTA) and produced by the Carolyn Campagna Kleefeld Contemporary Art Museum (the Museum); the Carpenter Performing Arts Center (CPAC); the Bob Cole Conservatory of Music; the School of Art, and the departments of Dance, Design, Theatre Arts, and Film and Electronic Arts. Themed around bringing communities together as campus repopulates, events will only be offered in full compliance with fluid COVID-19 safety guidelines and may transition between remote, hybrid, and in-person formats. Whether we are together in-person or gathering from a distance, we enrich our daily lives by actively interfacing with arts and culture.
