Art Education Student Safety Information and Form
All students involved in work within the School of Art Art Education classroom and studio spaces must complete the steps outlined below in order to use the facilities and equipment provided there.

- Please fill out and sign the School of Art Safety Instruction Verification and Agreement Form. (Required for ALL students)
N95 Dust Mask
If you believe you may use a disposable N95 Dust mask or know you will wear one, go through the Volunteer N95 Dust Mask Waiver before use. This waiver is required every semester and applies to all your enrolled courses in which an N95 is used.
