Welcome to the EE Forms page. 

Instructions for Filling out Form:

1. Click on the appropriate Forms tab
2. Select the link.
3. Forms will have contact information and submission instructions. 

Note:

• Please allow 5-7 business days for request processing.
• To check the status of your request, click the Review Document button in the email confirmation sent to you.
• A CSULB watermark indicates that the form is still under process.
• Please have your academic requirement, unofficial transcript, and transfer credit report (for transfer students only) available to be uploaded online.
• You will receive a copy of the completed form. If approved, you can enroll in open sections of the course. If denied, you will not be able to enroll and may need to see your advisor.
