No. The survey does not ask respondents to enter their name or employee ID. The only place staff names and ID are collected is the incentive form, which is separate from the UCLA survey system. There is no way for the research team to connect specific survey responses to the incentive form information.

Data will not be reported for groups with 5 or less responses. For example, if 4 men respond to the survey, all from the same department, results would not be reported for this group as it could make them identifiable.
Staff must complete the incentive form in order to receive the $25 in Beach Bucks applied to their CSULB ID card. If you missed submitting your information on the Thank you page at the end of the survey, please complete this alternative form to request the incentive.
We anticipate having the $25 in Beach Bucks distributed to CSULB ID cards in May 2020.
