Note: To meet CSU Liability and Safety/Risk Management requirements, the CCE utilizes an online service learning database. Administration of the service learning program and some related documents, including the Student Handbook, Community Service Learning Plan, Student Service Learning Agreement, and Student Emergency Contact Information are managed through the database. In order to utilize the database, students, faculty, and agencies must be registered users. If you are not yet a registered user, a CCE staff member will contact you regarding how to register and will also schedule an onsite tutorial of the database with you.

If you have any questions regarding this form, agency placement information, or the service learning orientation please contact us at (562) 985-7131.
